Job Title: Patient Care Coordinator
Job Overview:
Contemporary Staffing Solutions is seeking a Patient Care Coordinator to join our client in a role that supports key business operations and contributes to important organizational initiatives. This Remote temporary opportunity focuses on improving patient engagement, supporting healthcare quality initiatives, and helping individuals navigate important preventive and medication related care needs. The position offers close collaboration with healthcare professionals, administrative teams, and community resources while providing meaningful patient support through outreach and care coordination efforts. Candidates who are organized, service focused, and comfortable communicating with diverse audiences will be well positioned for success.
Compensation:
$24.00 - $25.00/hour
Work Schedule:
Monday through Friday, normal business hours
Key Responsibilities of the Patient Care Coordinator:
  • Coordinate outreach efforts to patients regarding medication compliance, preventive healthcare needs, and recommended follow up services.
  • Support communication between patients, healthcare providers, pharmacies, and other care partners to facilitate timely access to appropriate care.
  • Maintain accurate documentation within electronic healthcare systems and tracking platforms while ensuring data integrity and compliance requirements are met.
  • Analyze available patient information to identify care opportunities, address barriers to engagement, and assist with appointment scheduling and follow up activities.
  • Ensure adherence to privacy, security, and organizational policies while providing professional customer service and administrative support.
Qualifications and Skills for the Patient Care Coordinator:
  • High school diploma, GED, or equivalent required; Associate’s or Bachelor’s degree preferred.
  • Minimum of three years of experience within a physician office, hospital, pharmacy, health plan, or similar healthcare environment, along with customer service experience.
  • Strong verbal and written communication skills with the ability to effectively engage patients, healthcare professionals, and administrative staff.
  • Proficiency with Microsoft Office applications, electronic medical records, web based systems, and general office technology; ability to type at least 45 words per minute.
  • Working knowledge of medical and pharmacy terminology, healthcare quality measures, managed care concepts, and healthcare compliance practices.
