Case Manager



A diligent job-seeker is needed for the Investigator position with our client in Philadelphia, PA! As the Investigator, you will be responsible for providing intake, investigation, assessment and case management services for clients. This direct hire position offers $45k- $53k annually!



Responsibilities of the Case Manager Include:


  • Receiving and investigating reports of abuse, neglect, exploitation and abandonment of individuals age 60 and above.

  • Determining if report is substantiated or unsubstantiated and makes referrals to appropriate agencies and services

  • Assessing the need for emergency intervention

  • Accompanies and assisting police in delivery and enforcing court orders

  • Conducts an assessment and developing a care plan for client, family, friends and service providers

  • Documenting all case activity in accordance with State regulations, agency policies, and best practice.

  • Providing financial or surrogate management as appropriate

  • Coordinating case planning with other service providers and other service delivery systems

  • Arranges and conducts case conferences

  • Locating and/or mobilizing resources to meet clients needs

  • Participates in on-going problem-solving, program evaluation and development with all staff

  • Other duties as assigned

Required Qualifications :
Requirements of the Case Manager Include:


  • Three years experience in direct services

  • Bachelors Degree in Social Work or related social science

  • A valid driver's license and access to a fully insured care, and when necessary, transporting of clients

  • Evenings and weekends on-call via cell phone on a rotating schedule basis

  • Availability via cell phone at all times during scheduled working hours

  • Excellent verbal and written communication skills

  • Strong analytical and organizational skills
